As a registered psychotherapist in Ontario, commitment is made to protecting the privacy and confidentiality of your personal health information in accordance with the Personal Health Information Protection Act (PHIPA, 2004).

Collection and Use of Personal Health Information:

Personal health information, including your name, contact details, health history, and session notes, is collected only as necessary to provide therapy and related services.

Information may also be collected for administrative purposes, scheduling, billing, and to comply with legal or professional requirements.

Security and Safeguarding:

All written and electronic records are securely stored and restricted to authorized personnel directly involved in your care.

Electronic information is maintained on encrypted platforms compliant with PHIPA.

Consent and Disclosure:

Your personal health information will not be disclosed to third parties without your informed written consent, except where required or permitted by law (e.g., court order, risk of harm).

You have the right to access and request correction of your records.

Client Rights and Inquiries:

You have the right to ask questions about the privacy or security of your information or to submit a complaint.

Concerns may be addressed to Iris Glaser, or escalated to the Information and Privacy Commissioner of Ontario.

Breach Notification and Correction:

In case of a privacy breach, efforts will be made to contain the breach, notify affected clients, and take corrective action.

This policy is reviewed and updated regularly to comply with applicable laws and professional standards.
